By submitting any material to Medklinn Sinus Allergies Survey Giveaway, you affirm the following:

1. Introduction

These Terms and Conditions govern the participation in the Survey Giveaway organized by MedKlinn International Sdn Bhd  in collaboration with Child Specialist Clinic. By participating in the Giveaway, you agree to these Terms. Please read them carefully.

2. Eligibility

The Giveaway is open to all patients of the clinic who complete the required survey.
Participants must provide accurate and complete information, including their name, email, and contact number, in the survey form.
Only one entry per person per month is allowed. Multiple entries will be disqualified.

3. Survey Completion

Participants can complete the survey either electronically via an e-form or through a physical form available at the clinic.
For every 50 completed surveys, one winner will be selected, with a maximum of two winners per month.

4. Selection of Winners

Winners will be selected through a random draw from the pool of eligible participants.
The draw will take place monthly, and the winners will be notified via the contact information provided.
If fewer than 50 surveys are collected in a given month, the entries will be carried forward to the next month. However, the number of winners will remain capped at two per month. Each participant can only win once during the campaign period.

5. Prizes

Each winner will receive a MedKlinn Asens+20i Air+Surface Sterilizer. Prizes are non-transferable and cannot be exchanged for cash or other products.
We reserve the right to substitute the prize with another of equal or greater value in case of unavailability.

6. Data Collection and Privacy

By participating in the Giveaway, you agree to provide your personal information, which will be used solely for the purposes of this Giveaway and in accordance with our Privacy Policy.
Your information may be shared with MedKlinn for the purpose of prize fulfillment.

7. Liability

The Clinic and MedKlinn are not responsible for any technical errors, lost entries, or any other issues that may prevent participation in the Giveaway.

8. Changes to the Terms

We reserve the right to modify or terminate the Giveaway and its Terms at any time without prior notice. Any changes will be effective immediately upon posting the revised Terms on our website.

9. Governing Law

These Terms are governed by the laws of Malaysia. Any disputes arising from the Giveaway shall be subject to the exclusive jurisdiction of the courts of Malaysia.

11. Validation Process

All submissions will be validated to ensure the content adheres to the Terms & Conditions before the prize is granted to the participants.

12. License Grant

You grant Medklinn a non-exclusive, royalty-free, irrevocable, perpetual license to reproduce, disseminate, modify, and use your submission materials in whole or in part, in all media now known or later developed, at the sole discretion of Medklinn.
You acknowledge that this license is granted without compensation or financial benefit.

13. Usage Rights

Medklinn has the sole discretion to use the submitted material, in whole or in part, in any and all communication channels, including but not limited to television, radio, websites, social media, print and online publications, newsletters, etc.
The material may also be used in current or future promotional, educational, and exhibition purposes.

14. Publication Consent

You consent to Medklinn’s publication of your submission, including your first name, first initial of your last name, and your city and state of residence, on its website as part of the Sinus Allergies Survey Giveaway or other promotional programs.
Participation and submission are voluntary.

15. Responsibility

You are solely responsible for your submission and any consequences of submitting it.
Medklinn is not responsible or liable for any breach of these terms and conditions by you, claims by a third party that your submission violates their rights, or claims by you that a third party is misusing your submission.

16. Changes

Medklinn reserves the right to modify these terms and conditions at any time without prior notice.
